回腸膀胱瘻を来した悪性リンパ腫の1例

A CASE OF ILEOVESICAL FISTULA CAUSED BY MALIGNANT LYMPHOMA

　症例は57歳，女性。イレウスの手術時骨盤内に巨大腫瘤（bulky mass）を認め，生検で非ホジキンリンパ腫と診断された。CHOP-Bleo療法4クール終了後に回腸膀胱瘻を併発した。腸バリウム造影で瘻孔を確認し，回腸部分切除術，膀胱・子宮全摘術および回腸導管造設術を二期的に実施した。

　術後6年を経過した現在，腫瘍の再発・転移所見は認めない。

A 57-year-old woman with abdominal pain, sequential constipation and diarrhea since August 1989, was operat-ed because of ileus and was found to have a bulky mass in the pelvic cavity. She underwent transverse colostomy and biopsy of the tumor on October 13, 1989 and patho-logical diagnosis was malignant lymphoma. She was transferred to our hospital on November 30, 1989. CT and MRI revealed a large pelvic tumor adherent to adjacent organs.
