　32歳の不妊症男性。右鼠径ヘルニア手術によると思われる右精巣萎縮と左III度精索静脈瘤を認め，精液検査でほぼ無精子症状態であった。左高位結紮術を施行し，約1.5年で精液所見が正常に復し，人工授精で妊娠，分娩に成功した。しかし，3.5年後に再び無精子症にて左精索静脈瘤の再発を確認。今度は低位結紮術を施行したところ，精液所見が再び正常化し，8年後に挙児を得た。無精子症でも精索静脈瘤手術が有効であった1例を報告した。

Reported herein is a 32-year-old, virtually azoospermic infertile man with right atrophied testis and left severe varicocele. Following high ligation of left spermatic veins, he showed significant improvement of semen quality and finally obtained his wife's pregnancy and delivery assist-ed by conventional AIH. A couple of years later, he revealed severe left varicocele and virtually azoospermia, again. The low ligation of spermatic veins for recurrent severe varicocele induced normal semen quality and his wife succeeded second pregnancy and delivery.
