急性上腸間膜動脈塞栓症と腎梗塞同時発症の1例

A case of synchronous acute occlusion of the superior mesentric and left renal arteries

　56歳，男性。腹痛，背部痛を主訴に来院し，腎機能障害，白血球およびトランスアミナーゼの上昇を認めるため，腎梗塞の疑いで当科に紹介された。血管造影上，上腸間膜動脈と腎動脈の閉塞を認めた。抗凝固療法を施行し，上腸間膜動脈の血流は一時的に改善したが，腸管壊死が生じ，小腸部分切除を施行した。腎機能障害は遷延し，週1回の血液透析を施行中である。両疾患を生前に診断し得たのは本邦で2例目である。

A 56-year-old male with a chief complaint of abdominal and back pain was referred to our hospital under the clinical suspicion of renal embolism. The laboratory study showed leukocytosis and elevated levels of LDH, AST, ALT and serum creatinin. Angiography and CT scan revealed the occlusions of the superior mesenteric artey (SMA) and left renal artery with right renal atro-phy. The trans catheteral infusion of thrombolytic agents was performed immediately after the angiogra-phy.
