遊離小体と索状物を伴った小児精巣水瘤

A Case of Hydrocele Testis Concomitant with Two Corpora Libera and a String-Like Substance in a Three-year-old Boy

　3歳10か月の男児。右陰嚢腫大を主訴に，1996年6月に当科を受診し，精巣水瘤根治術を施行した。水瘤内には，白色嚢腫状と赤色充実性の2つの遊離小体と1本の索状物が存在していた。これらの構造物は，術前に陰嚢超音波検査で同定可能であつた。病理所見からは，遊離小体と索状物は水瘤壁が脱落して形成されたものと考えられた。嚢腫状腫瘤と索状物を合併した精巣水瘤は，本邦における文献上初めての報告例と考えられる。

A three-year-old boy presented with the chief complaint of right scrotal swelling that had been seen since birth. Ultrasound revealed right hydrocele testis where two corpora libera and one string-like substance existed. Winkelman operation disclosed several unordinary con-tents ; 1) a milky whitish soft mass, with surface partly calcified, and filled with crystal fluid ; 2) a reddish elastic hard mass ; 3) a cystic lesion on the wall of hydrocele ; and 4) a string-like substance, of which one end was con-nected with the wall of hydrocele and the other with a cystic lesion.
