血胸を続発した腎外傷

Hemothorax Secondary to Retroperitoneal Bleeding from Renal Injury:A Case Report

　35歳，男性。サッカーの練習中に右側腹部を強く蹴られ，右側腹部痛および嘔吐を主訴に受診した。CT上右腎裂傷を認め，即日当科へ入院となった。入院時の胸部X線では異常を認めなかった。保存的治療をしていたところ，4日後に血胸を認め，著明な後腹膜出血による血胸と診断した。経過観察のみにて血胸は徐々に改善し，40日後には完全に消失した。腎外傷を認めた場合は，血胸の出現に留意し，定期的に胸部X線を施行する必要があると思われた。

A 35-year-old man developed right flank pain and vomit-ing after receiving a kick during a soccer game. Computed tomography revealed right renal laceration and a large retroperitoneal hematoma but chest radiography did not show any abnormal finding. Four days after renal injury, chest radiography revealed a large pleural effusion in the right lung. Accorting to thoracentesis, we diagnosed it as hemothorax secondary to retroperitoneal bleeding from renal injury. The hemothorax gradually improved and disappeared forty days after renal injury.
