副腎血管腫性嚢腫

Adrenal Angiomatous Cyst

　症例は28歳，女性。突然右側腹部痛が出現し，近医を受診した。腹部CT検査にて右腎上方に占拠性病変が認められ，当院へ紹介となった。CT,MRI所見上，右副腎に嚢胞性病変が存在しており，一部実質性部分もみられた。内分泌学的検査では異常は認められず，非機能性副腎腫瘍と考えられたが悪性を否定できず，左副腎摘出術を施行した。病理組織学的診断は，Ⅷ因子免疫染色陽性の上皮細胞より構成される血管腫性嚢腫であった。副腎血管腫性嚢腫の報告は，自験例が本邦第2例目であると思われたが，稀な疾患であり，術前における腫瘍との鑑別は困難であった。

A 28-year-old female was referred to our clinic with the complaint of right abdominal pain. CT and MRI revealed the cystic lesion of the right adrenal gland, accompanied by a solid portion. Hormonal examinations showed no abnormality. Under the diagnosis of the non-functioning tumor of the adrenal gland, right adrenalectomy was per-formed. The histopathological finding was angiomatous cyst, because epithelial cells of the cyst were stained by factor VIII. In this case, it was not easy to distinguish cyst from neoplasm prior to a surgery.
