高カルシウム血症を呈した尿管移行上皮癌

Humoral Hypercalcemia of Malignancy Associated with Ureter Carcinoma:A Case Report

　症例は60歳，女性。1994年9月，肉眼的血尿にて当科を受診した。右尿管腫瘍の診断にて，10月19日に右腎尿管全摘除術，膀胱部分切除術を施行した。病理組織学的診断はTCC,G3,pT3,pN2であった。術後，MVAC療法を3コース施行した。1996年4月に再発し，1997年10月5日に癌死した。終末期の1997年9月に高カルシウム血症を呈し，血清PTHrPが高値を示した。病理解剖で得られた転移巣は移行上皮癌で，組織中のPTHrPも高値を示した。悪性腫瘍における高カルシウム血症は稀な病態ではないが，腎盂尿管癌や膀胱癌での報告例は少ない。また，組織型によって頻度が異なり，移行上皮癌で高カルシウム血症を合併する例は稀である。

A 60-year-old woman was referred to our hospital with a chief complaint of gross hematuria. We performed right nephroureterectomy under the diagnosis of ureter carci-noma. The pathological diagnosis was transitional cell carcinoma (TCC), G3, pT3, pN2, pV1, pL1. The tumor recurrence occurred 15 months later, and she died 3 years after operation. At the end of her clinical course the labora-tory data showed hypercalcemia and elevated serum PTHrP. Multiple organ metastasis was shown at autopsy, and pathological diagnosis of organ metastatic tumor was TCC. PTHrP levels of metastatic tumors were also elevated.
