腎摘出術14年後に膵転移が明らかになった腎細胞癌

Pancreatic metastasis occuring 14 years after nefrectomy for renal cell carcinoma

　63歳，女性。1985年5月右腎摘除術施行。病理診断は淡明細胞癌，G1,pT2NOMO。以後，当科外来にて定期的に経過観察していたところ，1999年6月腹部CTにて膵尾部に，造影される長径7cmの腫瘍を認めた。1999年8月膵体尾部切除術施行。膵組織像は淡明細胞癌，G1の所見であり，腎細胞癌の14年目の膵転移と診断された。術後，インターフェロンα療法を開始した。術後5か月経過した現在，再発は認めていない。

In May 1985, a 49-year-old woman underwent a right nephrectomy for renal cell carcinoma. She remained well on follow-up. In June 1999, when she was 64-year-old, abdominal CT revealed an enhanced solitary mass in the pancreatic tail. She underwent a distal pan-createctomy. Histological diagnosis of the specimen was clear cell carcinoma, metastatic from renal cell carci-noma. She is treated by interferon alpha and alive 5 months after the operation with no evidence of recur-rence.
