S状結腸癌による炎症性結腸膀胱瘻

A Unique Case of Sigmoidvesical Fistula

　S状結腸癌による炎症性結腸膀胱瘻の1症例を報告する。自験例はS状結腸癌の膀胱浸潤と考えられ，S状結腸切除，膀胱全摘除術，両側尿管皮膚瘻術を行った。組織学的には膀胱への癌の直接浸潤はなく，腸管周囲に炎症が生じ膀胱に波及して瘻孔を形成したものと考えられた。S状結腸癌の直接浸潤による瘻孔形成例の報告はあるが，癌の直接浸潤はみられず，二次的炎症により瘻孔を形成した症例は極めて稀である。

A 75-year-old male presented with ileus and pneumaturia and fecauria. Cystogram showed sigmoidovesical fistula. To-tal cystectomy and partial sigmoidectomy were performed under the preoperative diagnosis of invasion of sigmoid colon carcinoma to urinary bladder. Histopathological find-ings revealed no evidence of invasion except for abscess formed along the tract of fistula. It is suggested that sig-moidovesical fistula was due to unusual involvement of obstructive colitis secondarily developed by sigmoid colon carcinoma.
