尿膜管由来と考えられた印環細胞癌の1例

SIGNET-RING CELL ADENOCARCINOMA OF URACHAL TUMOR

緒言

　膀胱に発生する腫瘍の多くは移行上皮癌であり，腺癌の発生する頻度は1〜2％である^1）。膀胱に隣接する尿膜管の場合，腺癌が多数を占めるが発生数そのものが少ない。ただし，いずれの腺癌においても印環細胞癌（signet-ring cell carcinoma）が発生することは極めて稀であり，欧米文献を含め27例が報告されているにすぎない。今回われわれは膀胱に発生した尿膜管由来と考えられる印環細胞癌の1例を経験したので，臨床所見および若干の文献的統計考察を加えて報告する。

A 52 year-old man was admitted complaining of asymptomatic macrohematuria. He had noticed macrohematuria without irritability of the bladder since March, 1982. Cystoscopy revealed a non-sessile tumor with necrotic surface at the dome of the bladder. Biopsy demonstrated signet-ring cell adeno-carcinoma with normal bladder epithelium. Gastrointestinal fiberscopy and fluoroscopy were performed to rule out secondary tumor from gastrointestinal tract. However, no tumor could be found in any other organ.
