内視鏡的切開を行った前立腺部異所開口尿管結石

URETERAL STONE IN RIGHT COMPLETE DOUBLE PELVIS AND URETER: A CASE REPORT

　症例は右側腹部痛と頻尿を主訴とする48歳，男性。DIPで右完全重複腎盂尿管症と右上半腎所属尿管に12×6mmの下部尿管結石を認めた。右上半腎所属尿管は前立腺部尿道に異所開口しており，TULが施行できず，透視下に結石直上の前立腺部尿道粘膜をTUR器械にて縦切開を加え結石を摘出した。結石成分は蓚酸カルシウム結石で，術後1か月目のDIPでは右上半腎盂腎杯とその所属尿管の拡張は軽減し，自覚症状も消失した。

A 48-year-old man was admitted with complaints of right flank pain and pollakisuria. DIP revealed right complete doublepelvis and ureter, and ureteral stone in upper collecting system, which opend to the prostatic urethra. Therefore, we made transurethral endoscopic incision on the prostatic urethra just above the stone and accom-plished the stone extraction. A month after surgery, the dilated pelvis and ureter be came normall with the symp-tomatic improvement.
