多発性に硬化性腺症を伴う前立腺肥大症

A CASE OF PROSTATIC HYPERTROPHY SHOWING MULTIPLE SCLEROSING ADENOSIS

　69歳，男性。排尿困難にて発症した前立腺肥大症で，被膜下切除された前立腺内に多発する硬化性腺症を認めた。大きさは2〜14mm（平均径：6.8mm）で14病変を認めた。免疫組織化学的検索により，基底細胞と筋上皮細胞様紡錘形細胞に高分子量ケラチン（EAB903），アクチン，S−1OO蛋白が陽性に染まった。これらのマーカーは，小腺管の二層性の確認に役立ち，高分化前立腺癌との鑑別に有用である。

A 69-year-old man presented with symptom of urinary tract obstruction, and underwent suprapubic pros-tatectomy under the diagnosis of prostatic hypertrophy. Fourteen lesions of sclerosing adenosis were identified in serial sections of the whole resected material. The size of the nodules ranged from 2 to 14 mm (mean, 6.8mm). Immunohistochemistry utilizing keratin 903, muscle-spe-cific actin and S-100 protein, was useful in identifying basal cells and myoepithelial cells in sclerosing adenosis, thus verifying its benign nature and the useful-ness of these antibodies to distinguish this benign lesion from adenocarcinoma.
