　大便失禁を外科的に制御する順行性禁制浣腸法（antegrade continence enema：ACE）の臨床経験を報告する。本法の特徴は虫垂を右下腹部に固定し，患者がこのストーマから順行性に浣腸するものである。患者は二分脊椎症に罹患する17歳の男児で，術後の大便失禁はほとんど消失した。汎用されている逆行性洗腸法に比較し，短時間で大腸内容物を排泄でき，その効率も高いので，ACEは排便管理に有効と結論する。

We report our surgical experience of antegrade conti-nence enema (ACE) procedure, which aimed to surgi-cally control fecal incontinence. The surgery comprises anastomosis of the appendix to the right lower quadrant of the abdomen. A 17 year-old-boy suffering from myelodysplasia underwent this operation together with bladder neck sling and bladder augmentation. Post-oper-atively he became completely free from fecal soiling by performing ACE twice a week, ie. instillation of 100 to 150 ml of warm water through the appendiceal stoma.
