膀胱腫瘍に対するBCG膀注療法の経験

EXPERIENCE WITH INTRAVESICAL BCG THERAPY FOR PROPHYLAXIS OF SUPERFICIAL BLADDER CANCER

緒言

　表在性膀胱腫瘍に対して1976年Moralesら^1）がBacillus Calmette-Guérin　（BCG）の膀胱内注入療法を報告して以来，欧米を中心にその評価が高まつている^2〜9）。特に最近では上皮内癌（CIS）の治癒例や^5〜8），局所浸潤癌の治癒例^5,6）も報告されている。われわれも膀胱腫瘍に対して，Moralesらのレジメンに準じてBCG膀注および皮内接種（以下皮内接種も含めてBCG膀注と略す）を施行したので若干の知見とあわせて報告する。

Ten patients with superficial bladder cancer were treated with BCG for prophylaxis of tumor recurrence. The BCG therapy was initiated one to two weeks after TUR or biopsy of the tumor. One hundred twenty mg of BCG in 40 ml of physiologic saline was instilled into the bladder with concurrent administration of 12 mg of intradermal BCG, once a week for six times and once a month for ten times thereafter. Though five of these patients were with 14 episodes of tumor recurrence per 161 patient･month before the BCG therapy, only one tumor recurrence per 109 patient･month was observed after the BCG therapy.
