膀胱全摘8年後の尿管回腸導管吻合部再発

RECURRENCE OF TRANSITIONAL CELL CARCIOMA AT URETERO-ILEAL ANASTOMOSIS FIVE YEARS AFTER TOTAL CYSTECTOMY ; A CASE REPORT

　症例は79歳男性。1987年3月，肉眼的血尿を主訴にて当科初診。膀胱癌の診断のもと，膀胱全摘除術，回腸導管造影術を施行された。病理組織診断は移行上皮癌G2pTlで，再発を認めず，1990年2月より来院しなくなった。1994年12月に再び肉眼的血尿を主訴に当科を来院し，左尿管回腸吻合部に乳頭状腫瘍がみられ，左腎尿管摘除術，回腸導管摘出術，右尿管皮膚瘻造設術を施行した。病理組織診断は移行上皮癌G2だった。

A 79 year-old male visited our hospital with a chief complaint of macroscopic hematuria. He had a history of bladder tumor and had underwent total cystectomy with ileal counduit formation eight years ago. Histological diagnosis of the bladder tumor was TCC, grade 2, pT1. Cystoscopic observation revealed papillary tumor at left uretero-ileal anastomosis of the conduit. Then, left ne-phroureterectomy, resection of the conduit and right uretero cutaneostomy, were performed. Histological diag-nosis of the excised tumor was TCC grade 2 of the left ureter.

Neither recurrence nor metastasis have been observed yet.
