女子巨大膀胱結石の1例

A CASE OF GIANT VESICAL CALCULUS IN A FEMALE

　症例は43歳，女性。主訴は排尿困難。他院にて超音波検査で右水腎を指摘され当科を受診した。排泄性尿路造影で膀胱結石，膀胱憩室と診断した。膀胱切石術，憩室切除術を施行したところ術中右尿管瘤を認め，その切開術も加えた。結石は9×7.5×6.2cm，重量420gであった。膀胱憩室と尿管瘤以外に下部尿路の器質的，機能的異常は認められなかった。女性の巨大膀胱結石の本邦報告例では自験例は3番目の重量であった。

A 43 year old female was hospitalized with the chief complaint of dysuria for 3 years. Ultrasonography revealed right hydronephrosis.

She was diagnosed to have a vesical calculus and a vesical diverticulum by IVU. The cystolithotomy with diverticulectomy was performed.

The right ureterocele was discovered at the operation and its resection was done subsequently. The calculus was 420g (9×7.5×6.2cm).

This calculus was the third heaviest weight reported in Japanese literature.
