回腸を利用した代用尿管に発生した腺癌

LATE DEVELOPMENT OF ADENOCARCINOMA OF THE ILEUM FOLLOWING URETERAL SUBSTITUTION

　42歳，女性。難治性膀胱炎にて受診。30年前に回腸による左代用尿管形成術施行。膀胱鏡にて膀胱頂部の吻合部小腸側に広範な腫瘍を認め，膀胱全摘，代用尿管摘出，尿管皮膚瘻造設術を施行した。病理診断は回腸より発生した腺癌であった。尿路に利用した回腸に発生した腺癌の報告はきわめて稀である。

A case of adenocarcinoma occurring in the ileum 32 years after ureteral substitution is presented. Patientwas a 42-year-old female with the chief complaint of frequent micturiton. A non-papillary tumor was noted at the ileal site of the ileovesical anastomosis on the dome of the bladder by endoscopic examination. The lesion was biopsied and was shown to be an adenocarcinoma. This seem to be the 6th case with adenocarcinoma occurring in ileal site used as the urinary diversion in Japan.
