嚢胞内に出血した小児精巣上体嚢胞

EPIDIDYMAL CYST ASSOSIATED WITH INTRACYSTIC BLEEDING : A CASE REPORT

　症例は9歳，男児。受診数日前より左側陰嚢の疼痛を認め，さらに腫脹も伴ってきた。1993年9月に当院受診し，左側精巣捻転症の疑いにて緊急手術を施行した。術中所見にて，精巣上体嚢胞内の出血と診断され，これを切除した。急性陰嚢症を呈した精巣上体嚢胞内の出血の報告は筆者らが調べた限りでは自験例が2例目である。

A 9-year-old boy was admitted to Kusatsu Chuo Hospital because of acute pain and swelling in the left scrotum. Physical examination revealed tender scrotal contents of a small hen-egg size. An emergency operation was performed under the tentative diagnosis of torsion of the testis. However, the left testis was intact, where as a 3× 2 × 2 cm cystic mass was attached to the head of the epididymis. Diagnosis was the hemorrhagic epididymal cyst, which was removed en bloc without difficulty. Histologically, the cyst wall was composed of a fibroconnective tissue with interstitial hemorrhage.
