遠位尿管閉鎖症の1例

DISTAL URETERAL ATRESIA: A CASE REPORT

　生後23日の男子。主訴は腹部腫脹。生後腹部腫脹が持続するため当院を受診した。lVU，CTにて左巨大尿管，左腎機能低下の診断にて左ループ尿管瘻造設術を施行。術後左腎よりの尿流出はなく，腎機能廃絶と判断した。左尿管瘻造影では尿管下端は盲端に終わっており遠位尿管閉鎖症と診断し，左腎尿管摘出術を施行した。術中所見でも，左尿管は交差部下方で盲端に終わり，膀胱との連絡はなかつた。

The patient was a 23-day-old male, admitted to our hospital with the complaint of abdominal swelling. Urine and other laboratory examination were normal. Excretory urography (IVU) could not visualize the left kidney. Computed tomography (CT) showed left giant hydroureter and dysfunction of the left kidney. Although left loop ureterostomy was initially performed, urine flow from the left kindey was almost none. Left percutaneous pyelo-ureterography showed that the left ureter ended blindly. A clinical diagnosis of left distal ureteral atresia was made and left nephrectomy was performed.
