エンドメトリオーシスを伴つた遠位尿管閉鎖症

A CASE OF DISTAL URETERAL ATRESIA WITH ENDOMETRIOSIS

　19歳女性。左副卵巣嚢腫の診断にて開腹手術を施行。子宮，卵巣とも正常で，嚢腫は後腹膜腔に存在，口側には尿管遺残物と思われる管状構造物が連続，肛側は一部膀胱壁に癒着していた。嚢腫は約500mlの血性液で満たされ，上下端とも盲端で，左腎は認められず，病理組織学的には尿管と同様の構造を有し，嚢腫の一部にはエンドメトリオーシスと思われる組織が存在。エンドメトリオーシスを伴つた遠位尿管閉鎖症は本邦初例と思われる。

The patient, 19-year-old female, presented on July 1, 1986, with a progressively increasing lower abdominal swelling in one month duration. Before this, she was given a pill for 2 months. This was associated with no pains, and she did not have any urinary complaints. There was a large cystic mass in the left half of lower abdomen and sonograms showed 10 cm × 8 cm sized cystic mass. A tentative clinical diagnosis of It. paraovarian cyst was made and operation was performed.
