　患者は56歳，男性。1985年から多血症があり，1988年8月，肉眼的血尿を認め受診。ヘモグロビン23.2g/dl，ヘマトクリット70％，赤血球数712万/mm³だった。CTスキャン，血管造影で右腎下極に腫瘤を認め経腹的根治的腎摘出術を施行した。術後，ヘモグロビンなどは正常化した。腫瘍組織のエリスロポエチン濃度は正常組織に比べ高値だった。赤血球増加症を伴った腎癌の報告は1962年の田辺らの報告以来20例だが，エリスロポエチン産生腫瘍と証明されたものは12例である。

A 56-year old man with renal cell carcinoma had erythrocytosis that improved after right nephrectomy. He presented asymptomatic macroscopic hematuria. Erythrocytosis was a result of over production of erythropoietin (Epo) but serum level of Epo was normal. Direct evidence of Epo production in renal cell carcinoma was provided by higher Epo concentration in the tumor than normal renal tissue. This is the 20th docummented case of renal cell carcinoma with erythrocytosis and the 12th case of renal cell carcinoma producing Epo in Japan.
