　56歳，男性。主訴は便秘。右上腹部全体を占める巨大腫瘤を認め，精査の結果，右上腹部腫瘍と診断し，腫瘍摘除術を施行した。腫瘍組織は原発性後腹膜平滑筋腫であった。本症はきわめて稀な疾患とされ，本症例は本邦において30例目と考えられる。本症は平滑筋腫との鑑別がきわめて困難な場合があり，術後経過観察を十分に行う必要がある。

A 56-year-old man complaining of constipation was found to have a right abodominal tumor on ultrasonic examination. A retroperitoneal tumor was suspected by CTscan, MRI and angiography. The removed tumor 2,460 g in weight and 18 x 20 cm in size was soft. The cutsurfase was yellowish white in color with a dark brown lesion. Microscopic examination confirmed the diagnosis of smooth muscle tumor. This type of tumor is rare. Our case is the thirtieth in Japan. In this type of tumor, it is of great importance but often difficult to differentiate a benign tumor from a malignant one.
