マイトマイシンC膀胱内注入後に生じた萎縮膀胱

A CASE OF CONTRACTED BLADDER FOLLOWING INTRAVESICAL MITOMYCIN C THERAPY

　65歳，男性。表在性膀胱腫瘍切除後，腫瘍の残存が疑われ，マイトマイシンC20mgの膀胱内注入療法を計20回施行した結果，萎縮膀胱による，水腎，水尿管をきたしたため，膀胱全摘術および回腸導管造設術を施行した。摘出された膀胱の壁は著明に肥厚し，粘膜には出血性の潰瘍を認めた。顕微鏡的には，癌組織の残存は認められず，筋層に強い線維化が起こっていた。

A rare case of contracted bladder following intravesical mitomycin C therapy in a 65 y.o. male wasreported. After superficial papillary and non-papillary bladder tumors were resected transurethrally,mitomycin C 20mg in 20ml saline was instillated into the urinary bladder 20 times for six weeks. After abouttwo months, he complained urgent incontinence. After three months, computed tomography and excretoryurography showed contracted bladder, bilateral hydroureters and hydronephrosis. He underwent totalcystectomy and ileal conduit formation.
