症例は74歳，男性。関節リウマチに対し21年間シクロホスファミドの投与を受けていたが，出血性膀胱炎・萎縮膀胱により回腸導管による尿路変向術施行。その3年後，外尿道からの出血を主訴に来院。遺残膀胱に腫瘍を認めたためTUR-BTを施行したが，浸潤性膀胱癌の診断にて膀胱全摘術に至った。自験例は，本邦における28例目のシクロホスファミド誘発性と思われる尿路上皮癌症例である。

A 74-year-old man,who had received cyclophosphamide therapy for 21 years for the treatment of rheumatoid arthritis presented to our hospital complaining of the hemorrhage from his external urethral meatus. Cystoscopy revealed invasive bladder cancer and was transurethrally resected. The diagnosis was urothelial carcinoma,grade 2,pT2 and he underwent radical cystectomy. Several cases of cyclophosphamide-induced bladder cancer were reported. This is 28th cyclophosphamide-induced bladder cancer case in Japan.
