圧電方式による体外衝撃波を用いた尿管結石の治療

THE TREATMENT OF URETERAL STONES WITH THE EXTRACORPOREAL PIEZOELECTRIC LITHOTRIPSY

　1987年12月より圧電方式体外衝撃波砕石機を用いて尿管結石の治療を施行し，良好な結果を得たので報告する。対象は，上部から中部尿管結石の患者12名（17結石）であった。上部から中部尿管結石の治療の際，患側を下にした側臥位から背側に20°から45°傾けた体位で治療ベッドに寝かせた。結石の平均サイズは12.9×7.2mmであった。17個の結石中16個（94％）を完全破砕し，治療後1か月の時点で17個の結石中14個（82％）に完全排出を認めた。この体位は従来の仰臥位での治療に比して優れていることが確認できた。

From December 1987 to April 1988, 12 patients with 17 upper or middle ureteral stones were primarily treated with extracorporeal Piezoeletcric lithotripsy. In order to obtain clearer image of stones patients were fixed in stone-bearing-side-down lateral position with an angle 20 to 45 degree posteriorly. Before or after the initial trial of extracorporeal Piezoelectric lithotripsy, retrograde endoscopic procedures including stent placement could effectively move the calculi into the renal pelvis or collecting system in 2 out of 6 patients.
