　69歳，男性。左無痛性陰嚢内腫瘤で来科。腫瘤は空豆大で，精巣，精巣上体，精索とは離れて存在し，可動性があった。精巣固有鞘膜を開くと，固有鞘膜壁側板の内面に3.5×2.3×1.0cmの多房性嚢胞を認めたので摘除した。病理組織学的に，各嚢胞の境界は薄い線維性組織のみからなり，嚢胞上皮は多列円柱上皮であった。精巣垂の上皮も多列円柱上皮であることと，患側の精巣垂がみられなかったことから，ミュラー管由来の嚢胞と考えられた。

A 69-year-old man presented with a mass in the left scrotum. On palpation the mass was as large as a bean and separate from the left testis. epididymis or spermatic cord. A scrotal incision disclosed that the mass, measuring 3.5 × 2.3 × 1.0 cm, was a multilocular cyst arising from the parietal wall of the tunica vaginalis testis, and the left testicular appendix was absent. Pathological examination revealed that the septa between the cysts contained only fibrous tissue and the cysts were lined by ciliated pseudostratified epithelia resembling those of the testicular appendix.
