二重穿刺針を用いた膀胱頸部吊り上げ術

BLADDER NECK SUSPENSION USING DOUBLE NEEDLE APPLICATOR FOR FEMALE STRESS INCONTINENCE

　女子腹圧性尿失禁の12例に対して，Eickenbergの開発した二重針穿刺器具を用いた腟壁切開を行わない膀胱頸部吊り上げ術を行った。腟壁より二重針を直接刺入し，針先を恥骨後面に沿わせて恥骨上へと誘導し，1号のナイロン糸で膀胱頸部を挙上する方法である。術後，尿失禁は全例で消失し，従来のStamey法と比べて平均で約40分間の手術時間の短縮と出血量を大幅に減少させることができた。また感染や出血などの合併症を起こした例はなく，吊り上げに用いたナイロン糸やシリコンチューブは術後約6週後には腟粘膜下に埋没していた。この方法は腹圧性尿失禁の手術治療をより安全かつ簡便に行いうる方法であると思われた。

Twelve patients with stress urinary incontinence underwent endoscopic bladder neck suspension using an Eickenberg's double needle applicator, without making a vaginal wall incision. With this technique, the vaginal wall was punctured directly by the applicator. The tip of the needles were guided along the posterior side of the pubic bone to a point of suprapubic incision. Subsequently, the bladder neck was suspended with a nylon thread mounted on a small silicone tube.

After this procedure, urinary incontinence disappeared in all patients.
