　女性腹圧性尿失禁患者の手術療法としての内視鏡的針式膀胱頸部吊り上げ術は，その簡便さと優れた治療成績から現在広く行われるようになっている。しかし，術中合併症の一つとして，時々膀胱穿通がみられる。今回，腟壁より針を穿刺し，用指的にこの針を筋膜上に誘導する方法（digitally guided technique）で膀胱穿通を防止できたので，その手技を中心に報告する。

This report describes a new technique for performing bladder neck suspension on stress incontinent patients.

This technique aims to avoid bladder wall injury by stamey needle. No. 1 nylon monofilament suture is applied in a helical fashion from the vaginal mucosa deep into endopelvic fascia along the bladder neck laterally in a length of 2 cm bilaterally. A lower transverse abdominal 5 cm incision is then made above the upper border of the symphysis pubis through the skin and adipose tissue.
