著明な骨化と変性壊死を呈した無症候性副腎皮質腺腫

A CASE OF INCIDENTALLY DISCOVERED CORTICAL ADENOMA OF THE ADRENAL WITH MARKED OSSIFICATION AND NECROSIS

　78歳，女性．上部消化管X線検査にて右上腹部に径5cmの石灰化影を指摘された．精査により内分泌非活性右副腎腫瘍と診断し，摘出術を行った．手術中に血圧が上昇したために何らかのホルモン活性を有していたと思われた．病理学上は著明な骨化と変性壊死を伴う副腎皮質腺腫であった．

A 78-year-old woman with an incidental, markedly calcified right adrenal mass is reported. The mass was 5 cm in diameter. Endocrine data were all normal. The patient underwent a surgical excisison of the mass under a preoperative diagnosis of a non-functioning adrenal tumor. Manipulation of the mass at operation was followed by a sudden rise of the systolic blood pressure. After the extirpation of the mass, the blood pressure became immediately normal. The pathological diagnosis was cortical adenoma of the adrenal with marked ossification and necrosis.
