中等度の尿道下裂の手術法の検討—フリーグラフト法とダケット法の比較

Comparison Between Free Graft Method and Duckett Method for Moderately Severe Hypospadias Repair

　中等度の尿道下裂，主に陰茎部開口型の尿道下裂の修復術の術式として代表的な二術式（フリーグラフト法とダケット法）を経験し，手術結果の比較検討を行うとともに，それぞれの術式の特徴を考察した。両術式ともに，陰茎の背側包皮に横長の長方形を作図し，この長辺どうしを縫合し新尿道を作成して，索切除が終了した陰茎の腹側の尿道欠損部に補填する方法であるが，フリーグラフト法は作成した新尿道をいったん完全に遊離したのちに旧尿道口に吻合するのに対して，ダケット法は肉様膜を新尿道に付着させた状態で陰茎腹側に移動させて旧尿道口に吻合する点が異なる。手術結果として，初回手術成功率はフリーグラフト法が75％，ダケット法が88％であった。

We repaired hypospadias in 8 patients using a free graft of penile prepuce between October 1995 and July 1996, and in 8 others using an island flap technique between August 1996 and April 1997. We compared the results to determine which method is better for one-stage repair of moderately severe hypospadias. With the free graft methods, 6 of 8 patients were successfully repaired and one experienced urethral stricture and the other suffered from dehiscence of wound, while 7 of the 8 treated by the island flap method had successful repairs and urethrocutaneous fistula oc-curred in one patient.
