糸球体病変を伴つた尿道下裂

A CASE OF PROXIMAL HYPOSPADIAS COMPLICATED WITH GLOMERULAR DISEASE

　高度の尿道下裂に著明な蛋白尿を合併した症例を経験し，内分泌学的検索および腎生検を施行した。前者では軽度のアンドロジェン分泌障害パターンを示し，後者では糸球体の硬化像を中心とした様々な組織像がえられた。近年，男子仮性半陰陽にWilms腫瘍，腎炎を合併した症例はひとつの症候群としてとらえられるようになつてきており，特に近位例の尿道下裂症例を扱う際には，腎病変の有無も念頭に置く必要があることを強調した。

We experienced five year-old boy with proximal hypospadias and severe proteinuria. Endocrino-logical examination of the pituitary-gonadal axis disclosed the relative defect of androgen production compared with control. Renal biopsy revealed glomerulosclerosis with the thickening and electron dense deposit in basement membrane of glomerulus. The fluorescent antibody technique proved twice-positive IgM and C1q deposit, and positive IgA and fibrinogen deposit in the mesangium. Attention should be paid to the children with proximal hypospadias with regard to degenerative renal disease.
