精索腫瘤を形成したマンソン孤虫症

A CASE OF SPARGANOSIS MANSONI PRESENTING AS A MASS IN THE SPERMATIC CORD

　89歳男性，右陰嚢内腫瘤を主訴として来院した。精索腫瘍を疑い高位除睾術を施行した。摘出標本は栂指頭大の嚢胞と，精管に沿つて小豆大から大豆大の硬結を数個認めた。組織学的には硬結内にマンソン裂頭条虫の幼虫と思われる虫体を認め，マンソン孤虫症と診断した。ヘビの生食の既往があるため，それが感染原因と思われた。マンソン孤虫症は本邦に多く見られるが，精索腫瘤を形成することは稀で，本邦7例目と思われる。

A 89-year-old man presented with a painless mass in the right scrotum. With suspision of a malignant tumor, an orchiectomy was performed. The mass as large as thumb-tip was located at the right spermatic cord. Histological examination showed a worm body suggestive of Plerocercoid of Diphyllobothrium erinacei. The patient had a recent history of eating uncooked snake. Sparganosis mansoni in the spermatic cord is rare. This patient is the seventh case reported in the Japanese literature.
