膀胱粘膜による尿道下裂の一期的尿道形成術

REPAIR OF HYPOSPADIA USING BLADDER MUCOSA

　本法は先天性尿道下裂，尿道欠損に対して遊離膀胱粘膜を管状に縫合し，新しい尿道を形成する一期的尿道形成術である。本法の要点は陰茎腹側筋膜と海綿体白膜間を注意深く剥離すること，索を完全に切除すること，陰茎腹側に十分な皮膚を残すこと，および膀胱粘膜を正確に採取して尿道欠損部に遊離移植することである。本法の適応年齢は8〜10歳ごろが望ましい。術後尿道内カテーテル留置は行わない。術中，新尿道口からストレプトマイシン（粉末）を1g注入する。一期的尿路変更として恥骨上に膀胱瘻を作成し，2〜4週後にこれを抜去する。術中は創の腫脹，出血，ドレナージに注意する。適量の女性ホルモンと有効抗生物質の使用によつて術後成績を高めることができる。

We introduced a one-stage urethroplasty for hypospadias using a free flap of the bladder mucosa. A mucosal flap taken from the bladder was tubularized and was placed where chordee had been excised, and was anastomosed to the urethra. No catheter was placed in the urethra postoperatively. One g. of streptomycin powder was instilled in the urethra and a temporary cystostomy was made for 2-4 weeks. Fifty-nine patients aged 8-10 years were operated with satisfactory results.
