ループストーマによる回腸導管尿路変更術

LOOP STOMAS FOR ILEAL CONDUIT URINARY DIVERSION

　ループストーマによる回腸導管造設術では，通常行われているエンドストーマによるものに比べて，導管口狭窄が起こりにくいとされている。この事象を，術後短期間でストーマの狭窄を来しやすい生犬を用いた実験で検討した。さらに臨床例に施行し，臨床上の問題点にも検討を加えた。生犬に施行したループストーマによる回腸導管造設術では，術後100日目の時点で導管口狭窄を認めず，導管の排尿状態も良好であつた。臨床例においても，エンドストーマによる回腸導管と比較して，臨床上の問題点は少なかつた。以上の結果より，回腸導管による尿路変更術には，ループストーマを適用することが推奨された。

The advantage of ileal conduit urinary diversion with loop stomas in terms of stomal obstructionover usual end stomas has been reported. We evaluated loop stomas experimentally and clinically.Ileal conduits with loop stomas were constructed in three mongrel dogs anastomosing the right ureteralone.On the 100th post operative day,condition of the stomas and urinary dynamics of the ileal conduits were evaluatd.No stomal obstruction was observed in the experimental animals and urinary dynamics of the ileal conduits were favorable.
