泌尿器科入院患者の精神障害—特にせん妄にっいて

MENTAL DISORDERS OF THE PATIENTS IN UROLOGICAL WARD

　泌尿器科入院患老の精神障害について検討した。1981年1月から1986年6月の5年6カ月の間に精神科医の診察を依頼した患者は30名で，入院患者の1.4％であつた。このうち，せん妄状態にあつたのは20名であつた。入院時あるいは入院後にせん妄状態となつた13例中9例では，泌尿器科疾患に対する根本治療ができなかつた．手術後にせん妄状態となつた7例中3例が死亡した。せん妄状態となつた患者の予後は，特に術後では悪いので，原因，誘因の検索とともに，精神科医による速やかな治療が必要である。せん妄患者の大部分は痴呆を合併するので，老年者に対しては入院時に痴呆のスクリーニソグが必要である。

From January 1981 to June 1986,psychiatric consultation was required in thirty patients who were admitted to urological ward of Sunagawa City Medical Center,for their mental disorders.20 patients were diagnosed as delirious state and 13 of them were delirious on admission or became delirium soon after admission, and in 9 of them, we could not perform the planned urological therapy. Seven patients experienced postoperative delirium,and 3 of them died of acute renal failure,sepsis or hypernatraemia.
