弁式精管吻合術の18例

VASOVASOTOMY FLAPS ANASTOMOSIS:ENCLOSED THE 18 CASES REPORT

　精管結紮後の18例に対して，Fitzpatrickの術式に従つて，手術顕微鏡（倍率10倍）下のマイクロサージェリーによつて，縫合糸が精管壁を貫くことなく，また，使用する弁の大きさを適宜調節し，腔内に支持具を残さずに精管を再吻合した。その結果，術後3〜14週の精液検査で，不成功1例，精子数のやや低かつた（1,400万／ml）1例を除き，すべて6,000万／ml以上，精子活動率は60〜80％に達し，術後6,9,12カ月日の経過観察の結果では妊娠率61％に達した。

The reports analysed the 18 cases of anastomosis of vas deferens by Fitzpatrick operation.The operation principal is to use"double-swedged continuous suture"or"symmetrical modular suture"under the operating microscope.The stitch should never go through each of the side walls of the lumen and without luminal splints.In 3~4 weeks after operation,the sperm analysis was made.Except for one failure case and one with lower sperm counts (14 million/ml),most patients had more than 60 million/ml of sperm counts and had 60~80% sperm mobility rate.In 6,9 and 12 months,their pregnancy rates rose up to 61%.
