Cohen術式による膀胱尿管逆流防止術の成績

RESULTS OF ANTIREFLUX OPERATION WITH COHEN PROCEDURE FOR PRIMARY VESICOURETERAL REFLUX

　原発性膀胱尿管逆流97尿管に対しCohen法による逆流防止手術を行い，その成績をPolitano-Leadbetter法（P-L法）76尿管のそれと比較検討した。その結果，手術成功率は両者とも97％前後を示し，満足すべき成績を得た。しかし，Cohen法はP-L法に比し手技が容易で安全であり，そのため術後入院期間が短縮されること，また新尿管裂孔を作成する必要がないため，術後上部尿路に及ぼす影響が少ないこと，などの利点を有することがわかつた。

The Cohen transvesical antireflux procedure achieved a success rate of 96.9 per cent in 97 ureters, almost equal to the results of the Politano-Leadbetter technique in our previous series. The Cohen procedure, however, is technically simpler and safer, resulting a lesser operation time and blood loss, shorter period of ureteral stenting, urethral catheterization and patient's hospital stay. Without need of a new ureteral hiatus creation in the bladder wall, no ureteral obstruction was observed 6 months or later postoperatively in this series.
