胸腹部連続正中切開法の経験

CONTINUOUS THORACO-ABDOMINAL MIDLINE INCISION: ITS EXPERIENCES IN TWO CASES

　われわれは両肺と後腹膜に転移を有する睾丸腫瘍患者，および広範な下大静脈腫瘍塞栓を伴つた左腎腫瘍患者の2例に対し，胸腹部連続正中切開を用いた手術を行つた。その結果，本術式には一般の上腹部正中切開に比べより広い後腹膜の視野が得られる。また，胸腔内と後腹膜，あるいは大静脈と両腎の手術を一つの視野で同時に行えるなどの利点があることを確認した。

We used the continuous thoraco-abdominal midline incision with median sternotomy in two cases, one with large retroperitoneal and bilateral pulmonary metastases of testicular tumor and the other with a left renal carcinoma with high grade vena cava invasion. This approach enabled simultaneous exposure of thoracic and retroperitoneal space and also provided larger working space in the upper retroperitoneal field. In addition, sternotomy could be relatively easily performed. Thus, it appears this incision of great benefit in similar cases as presented here.
