経皮的腎尿管結石摘出術における尿管結石の腎盂へのおしもどしの難易度

COULD PREOPERATIVE IVP ASSESS WHETHER MIDDLE AND UPPER URETERAL STONES CAN BE PUSHED BACK INTO THE RENAL PELVIS BY FLUSHING OR CATHETER MANIPURATION?

　上部および中部尿管に位置する結石をPNL　（経皮的腎結石摘出術）によつて治療する場合，結石を腎盂へおしもどすことができるか否かがこのPNLの難易度を決めることが多い。今回われわれは術前の腎盂撮影上，結石の部位，形態，大きさ，存在期間，および結石直上部の尿管の形態の5つの要因別に結石の腎盂へのおしもどしの難易度を検討した。その結果，術前のIVPにおいて結石直上部の尿管にくびれや造影剤の陰影欠損がみられる症例ではおしもどしが難しく，他の要因については差はみられなかつた。

In the treatment of upper and middle ureteral stones, our selection of percutaneous nephrolithotomy or transurethral lithotripsy largely depends on whether the stones can easily be pushed back into the renal pelvis by simple flushing or catheter manipulation. Studies were made in 29 cases with 32 ureteral stones on whether or not the degree of difficulty in mobilizing stones into the renal pelvis could be predicted by the findings of preoperative IVP.
