膀胱癌の浸潤度判定—CTとエコーの比較を中心に

DIAGNOSIS OF PREOPERATIVE STAGING OF BLADDER CARCINOMA BY CT AND ULTRASONOGRAPHY

緒言

　膀胱癌の治療法の選択にあたり，その深達度の判定，とりわけ膀胱壁の浸潤度の判定は，最も重要な因子と思われる。膀胱癌取扱い規約^1）では臨床的深達度判定法として，膀胱撮影，麻酔下での双手診，血管造影，CT，超音波断層法（エコー），リンパ管造影が挙げられている。これら判定法は，いずれも高い正診率を認めているが，各々別個に評価をなされていることが多い。今回著者は患者に対する侵襲度，および判定の正確さを考えて，最も優れていると思われるCTとエコーを比較検討し，この成績を報告する。

The preoperaive staging of urinary bladder carcinoma is extremely important for the choice of treat-ment modalities. The stage of urinary bladder carcinoma was determined by CT and transurethral ultra-sonography (Echo) and compared with histological stage in 62 patients. The overall accuracy rate of CT and Echo was 79.6% and 80%, respectively. The diagnosis of CT tends to be of overstaging, while the diagnosis of Echo tends to be of understaging. The diagnosis of CT and Echo was in good accordance each other at 67.6%.
