膀胱全摘除術不能の進行膀胱癌に対する術中照射術

INTRAOPERATIVE IRRADIATION FOR ADVANCED BLADDER CARCINOMA

緒言

　進行膀胱癌は，一般に膀胱全摘除術が最良の治療法と考えるが，尿路変更を拒否する患者や，高齢，全身衰弱，あるいは合併症のために膀胱全摘除術が不可能な患者に対しては，膀胱部分切除術，放射線外照射療法を行うことになる。しかし，進行膀胱癌に対する膀胱部分切除術や，外照射療法は必ずしも成績はよくない^1,2）。

　われわれは，進行膀胱癌に対する膀胱全摘除術の次善の治療法として，術中照射術を施行して，いまだ症例は少ないが，成績良好の結果を得たので報告する。

Intraoperative irradiation was performed on 9 patients with advanced bladder carcinoma (T2 : 1, T3 : 7, T4 : 1) because 3 patients wanted to spare the bladder function and 6 were considered to be too old or in too severe condition to recieve cystectomy. On 6 patients intraoperative irradiation was performed after tumorectomy and 3 patients received the radiotherapy focused on trigonum and internal urethral orfice after subtotal cystectomy with uretero-cutaneostomy.
