まえがき

　陰嚢内容の急性有痛性病変は，いわゆるacutescrotumと総称されている。陰嚢内容は表在性臓器であるにもかかわらず，急性病変では腫脹および疼痛が著しく，触診によつても十分な所見をうることが困難なことがあるので，その鑑別診断にはしばしば困惑することがある。このような時に超音波検査法を行うと，これら病変の診断に優れた効力を発揮することができる^1〜5）。

　われわれは，過去11年間に228例の急性有痛性陰嚢疾患に超音波診断を行い良好な臨床成績を挙げることができた。今回は，超音波断層法と超音波ドプラ法を用いたわれわれのacute scrotumの診断法について述べる。

Combination of real-time ultrasonotomography and ultrasonic doppler method was used for the diagnosis of the acute scrotum and has shown excellent results. During the last 11 years, ultrasonic diagnosis was carried out in 228 cases of the acute scrotum including 147 acute epididymitis, 16 acute orchids, 36 testicular torsion, 5 appendix testis or epididymis torsion, 22 testicular trauma and 2 other diseases. Results showed that accuracy rate of diagnosis was 100% in epididymitis, and orchitis, 94% in testicular torsion, 40% in torsion of appendix, 95% in testicular trauma, 100% in other diseases and the total accuracy rate was 97%.
