膀胱部分切除術の検討—再発後，膀胱全摘術を施行した症例の治療成績

CLINICAL AND PROGNOSTIC STUDIES ON 25 CASES WITH BLADDER CANCER WHO UNDERWENT TOTAL CYSTECTOMY DUE TO RECURRENCE AFTER PARTIAL CYSTECTOMY

緒言

　膀胱癌は泌尿器悪性腫瘍の過半分を占め，近年さらに増加の傾向にある。しかし，その治療法については，種々の形式のものが工夫されてはいるが，多くの問題を有している。

　当センターでは，他の医療施設にて，TURや膀胱部分切除術を受け，再発後に来院する症例が多い。今回，膀胱癌で膀胱全摘術を施行した症例のうち，膀胱部分切除術の既往を有する症例につき治療成績をまとめ検討を加えたので報告する。

Clinical and prognostic studies were performed on 25 cases who were treated by total cystectomy with recurrent bladder cancer after partial cystectomy: Following results were obtained:

1) The 5 years survival rate was 51% for all patients, 67% for stages Tis-T1 and 15% for stages over T2 diseases.

2) The low grade, low stage carcinoma had longer intervals between partial cystectomy and total cystectomy, and better prognosis than the high grade, high stage (over T2) carcinoma.

3)In one of 25 cases, tumor dissemination in the abdominal operation wound due to partial cystec- tomy was seen.
