膀胱腫瘍に対する膀胱部分切除術の成績

PARTIAL CYSTECTOMY FOR BLADDER CANCER

　膀胱部分切除術が行われた原発性膀胱移行上皮癌患者27例について，その転帰を調査し，これを規定する病理組織学的因子について検討した。術後の膀胱内再発は，12例（44％）に認められ，組織学的深達度，または異型度が高度なほど早期から高率に認められた。腫瘤形成性腫瘍周囲粘膜に上皮内癌を認めた4例はすべて2年以内に膀胱内再発をきたした。Kaplan-Meier法による5年生存率は68％で，　Iow grade症例てはhigh grade症例に比較して有意に生存期間が長かった。また，膀胱部分切除時high gradeで膀胱内再発をきたした症例では，癌死にいたる症例が多く認められた。

We reviewed 27 patients who underwent partial cystectomy in the treatment of primary transi-tional cell carcinoma of the bladder, and studied histopathological factors affecting clinical course. Intravesical recurrence was seen in 12 patients (44%). Patients with high stage or high grade carci-noma frequently recurred in the early postoperative periods. All 4 patients who had carcinoma in situ around exophytic lesion recurred in the bladder within 2 years.
