下大静脈切除術を要した右腎細胞癌の1例

SUCCESSFUL REMOVAL OF A RENAL CELL CARCINOMA WITH VENA CAVAL RESECTION: REPORT OF A CASE

緒言

　腎細胞癌による下大静脈栓塞は諸家により3〜10％^1〜4）の頻度で発生すると報告されているが，現在のところそれに対する唯一の根治的療法は腎摘除ならびに完全な血栓除去である。わが国ではそういう症例に対して下大静脈切除術を行なつた報告は極めて少ない。最近われわれは下大静脈切除術を要した右腎細胞癌の1例を経験したので，若干の文献的考察を加えて報告する。

A 53-year-old man was admitted to our hospital with complaints of painless hematuria on April 21, 1980. Roentgenographic examination revealed a right renal tumor extending into the vena cava approximately 7cm above and 3cm below the renal vein. The main mass of intracaval tumor was extracted without difficulty but since it was noted to be adherent to the posterior caval wall, the infra-hepatic vena cava was excised. Ligation of left renal vein and routine radical nephrectomy was performed. Histopathologic diagnosis was clear cell carcinoma.
