泌尿器科領域におけるβ2microglobulin（BMG）測定の意義

SIGNIFICANCE OF β2-MICROGROBULIN IN UROLOGIC FIELD

緒言

　β₂　microglobulin　（以下BMG）は，1968年Berggardら^1）により腎疾患患者の尿より分離された分子量11,600の低分子量の蛋白質である^2）。BMGはリンパ系細胞により産生され^3），正常人の血清，尿，髄液などに存在し，免疫学的にも注目されている^4,5）。低分子量のため糸球体で完全に濾過され，尿細管でほとんど100％再吸収，異化される。この性質を利用し，1969年，Petersonら^6）が初めて腎疾患の診断に応用して以来，ひろく研究，報告がなされている^7〜9）。近年，BMGは腎疾患のみならず，神経疾患，悪性腫瘍，リウマチ性疾患などでも異常を示すことが知られ^10），この方面でも応用が試みられている。

　今回，泌尿器科領域におけるBMG測定の意義について若干の検討を行なつたので，ここに報告する。

From August 1979 to July 1980, we evaluated serum and urine β₂-microglobulin (sBMG and uBMG) in 125 patients (49 males and 76 females). The ages of the patients were from 15 to 89 (average 43.8).

sBMG was proportional to serum creatinine, and uBMG was an excellent indicator of renal tubular disorders, especially in the case of pyelonephritis and obstructive nephropathy. In addition, correlation of cleatinine clearance (Ccr) to sBMG was expressed as follows.
