膀胱腫瘍に対する超選択的膀胱動脈塞栓術の経験

SUPERSELECTIVE CATHETERIAL EMBOLIZATION FOR MASSIVE HEMATURIA OF BLADDER TUMOR

　最近血管カテーテル法の進歩は著しく^1,2），Transcatheter Therapeutic Arterial Embolization（以下TTAE）が各種疾患に対して行なわれるようになつた。泌尿器科領域では，腎癌，腎動静脈瘻などに対して数多く行なわれている^3〜7）。

　膀胱腫瘍に対しても，最近では超選択的に上もしくは下膀胱動脈を造影することが行なわれるようになり，それによつて診断面にとどまらず選択的に抗癌剤や塞栓物質を支配動脈に注入して止血のみならず腫瘍の縮小を図るという治療面への応用が可能となつた。

A 61-year-old male, who had partial cystectomy twice in the past at the other hospital, was admitted to our hospital with recurrent bladder tumor associated with edema of the both lower extremities and a huge Virchow's lymphadenopathy.

Cystoscopy revealed wide-based nodular growths occupying entire right half of the bladder wall.

On the 10th hospital day, the patient started to have massive hematuria resulting urinary retention and frequent episodes of catheter obstruction in spite of the continuous irrigation.
