肉眼的血尿を主訴としたIgA腎炎の1例

A CASE OF IgA NEPHROPATHY WITH GROSS HEMATURIA

緒言

　IgA腎炎は1968年Berger^1）によつて，nephro-pathy with mesangial IgA-IgG depositsと題して，初めて報告された慢性に経過する糸球体腎炎の1型であり，しばしば反復性肉眼的血尿がみられる腎炎である。そのため，いわゆる肉眼的血尿を主訴として泌尿器科を訪れる患者の中にIgA腎炎患者が含まれている可能性がある。最近，われわれは肉眼的血尿を主訴として来院した33歳男子のIgA腎炎の1例を経験したので報告する。

On February 11, 1977, a 33-year-old man visited our out-patient clinic complaining intermittent gross hematuria and common cold like symptoms. On laboratory examinations there was no abnormality except for persistent microscopic hematuria, proteinuria and high erythrocyte sedimentation rate. A series of KUB, IVP and cystoscopic findings was negative. An open renal biopsy revealed IgG and IgA deposits in mesangium with hypercellularity which was compatible with mesangial IgA nephropathy.

It would be essential for urologists to recognize a possibility of gross hematuria due to IgA nephropathy.
