Regional Renal Hypothermiaによる腎結石の手術

SURGICAL MANAGEMENT OF RENAL CALCULI USING REGIONAL RENAL HYPOTHERMIA

緒言

　腎結石の手術は，その大きさ，部位などから，腎実質に切開を加える腎切石や腎部分切除などが必要な例がある。この場合，失血を防ぎ，無血で明瞭な視野を得るために腎茎血管の遮断が行なわれるが，阻血時間が長いと，その結果として腎機能障害が生ずる危険がある。一方，結石を完全に除去し，また必要により腎杯の形成を行なうためには十分な時間が必要である。

　阻血時間を延長させる手段としては，従来よりhypothermiaが施行されてきたが，われわれは簡単で，どんな手術室でも行なえるin situのhy-pothermia法で腎結石の手術を行ない，良好な治療成績を得た。本法の手技についてのべるとともに，その手術成績を報告する。

In 6 cases of multiple renal calculi, nephrolithotomy was carried out successfully under hypothermia by surface cooling.

After intravenous administration of 20g of mannitol, followed by 1,000 units of heparin into the renal artery, the renal artery was occluded using a vascular clamp. Then, the kidney was surrounded by a plastic sheet and the space between the kidney and the plastic sheet was filled with saline slush. Renal operation was performed with the kidney almost immersed in the saline slush.
