右側にみられた卵巣静脈症候群の1例

A CASE OF THE RIGHT OVARIAN VEIN SYNDROME

緒言

　妊娠中に種々の原因により上部尿路が拡張し，しかも右側に圧倒的に多いことはよく知られた事実である。1964年，Clark^1）は分娩後も右卵巣静脈の拡張が退縮せず，右尿管への圧迫が持続し，右尿管における尿流通過障害を呈する病態を彼の経験した130例の詳細な検討から"The right ovarianvein syndrome"として報告した。最近，われわれもClarkのいう症候群と同一の病態を呈し，手術的に確認しえた1例を経験したので，若干の文献的考察とあわせて報告する。

A 28-year-old woman visited our clinic on January 14, 1977, complaining of gross hematuria and vesical irritability. Excretory urogram showed the dilatation of right renal pelvis and ureter, which subsided abruptly at the level of S-1. Retrograde pyeloureterogram showed right ureteral filling defect presumably caused by the common iliac artery. On November 1, 1977, exploratory operation was performed. Dilated right ureter was entangled with the engorged ovarian vein and the ureter was obstructed by the vein on the common iliac vessel.
